The Livingston County Department of Health is holding a Moderna vaccine booster clinic at the Avon Middle School on Clinton Street on Jan. 7 from 9 to 11 a.m.
The clinic will be held in the middle school gymnasium and is open to the public. People are asked to park at the southernmost end of the middle school parking lot that’s closest to the school and enter through the auditorium entrance.
Because Jan. 7 is a scheduled late arrival day for Avon students, instruction will not be impacted by the booster clinic.
To receive a booster, individuals must have been fully vaccinated for at least six months and be over the age of 18.
Those wishing to receive a booster must also bring a photo ID and the vaccination card they were given when they received their first vaccine dose. Masks are required for the duration of the visit.
Those wishing to receive a booster must pre-register and bring a completed COVID-19 vaccine consent form and a completed registration form with them. The deadline to register is Jan. 6.
